เงื่อนไขการใช้บริการ

• ราคาพิเศษนี้ ขอสงวนสิทธิ์เฉพาะ Areeya Family เท่านั้น
• วัคซีนไข้หวัดใหญ่ ต้องฉีดในวันเดียวกันกับวันที่เข้ารับการตรวจแพ็กเกจนี้เท่านั้น และไม่สามารถมอบสิทธิ์ให้ท่านอื่นใช้แทนได้
  * วัคซีนไข้หวัดใหญ่ ไม่แนะนำให้ฉีดในขณะที่มีไข้ และไม่แนะนำในผู้ที่ฉีดวัคซีนไข้หวัดใหญ่เข็มล่าสุดใน 10 เดือนที่ผ่านมาและห้ามฉีดในผู้ที่มีประวัติแพ้ไข่ไก่หรือส่วนประกอบของวัคซีนชนิดนี้อย่างรุนแรง

• แพ็กเกจนี้ขอสงวนสิทธิ์ไม่สามารถระบุแพทย์ ไม่สามารถเปลี่ยนแปลงรายการตรวจ และไม่สามารถเพิ่มรายการตรวจอื่น ๆ ได้ โรงพยาบาลขอสงวนสิทธิ์ไม่คืนเงินในกรณีที่ไม่รับรายการตรวจใดรายการหนึ่ง
• ผู้เข้ารับบริการ จะได้รับผลการตรวจวิเคราะห์ทางห้องปฏิบัติการ ผ่านทาง Line @bnhhospital เท่านั้น (ไม่มีรายงานผลในรูปแบบกระดาษ) กรุณา Add Line และยืนยันตัวตนเพื่อรับผลตรวจ
• เนื่องจากแพ็กเกจนี้ไม่มีการตรวจร่างกายโดยตรงกับแพทย์ จึงไม่สามารถออกใบรับรองแพทย์ได้ ซึ่งแพทย์สามารถออกเป็นใบคำแนะนำจากแพทย์ (Doctor’s Recommendation) แทน โดยมีค่าใช้จ่ายเพิ่มเติม 200 บาท
• ท่านสามารถใช้บริการได้ตั้งแต่วันนี้ – 31 ธันวาคม 2567

• The influenza vaccine must be injected on the examination day only and cannot be transferred to another person.
  *Flu vaccine is not recommended to inject while having a fever and not recommended for people who have had their most recent flu shot in the past 10 months. If you have any severe, life-threatening allergies or ever had a life-threatening allergic reaction after a dose of flu vaccine, or have a severe allergy to any part of this vaccine, including an allergy to gelatin, antibiotics, or eggs should not get influenza vaccine.
• These special prices are reserved only for those who purchase online and make an appointment 24 hours prior to using the service.
• The services are available at the Emergency Department (ER), 1st floor. The results will be interpreted by a doctor via teleconsultation only.
• These packages reserve the right to choose a doctor, to change the examination list items, or to add any other examinations. The hospital reserves the right not to refund if not accepting any of the examination items.
• Health check-up results will be provided via the BNH’s LINE account @bnhhospital exclusively and self-verification is required to access the results. (Printed report is not available)
• Due to these packages do not include a direct physical examination by a doctor, therefore a medical certificate cannot be issued. A doctor’s recommendation certificate can be substituted for an additional charge of 200THB.

Preparation

• Please do not consume any food and drink for 10-12 hours (except sips of water), as well as alcoholic beverages for 72 hours before the test.
• For women, please avoid urine examination during the menstrual period. The sample can be collected at least 3-5 days after the menstrual period.
• For imaging diagnosis, PLEASE NOTIFY our staff in advance if you are pregnant or may be pregnant.
